● I acknowledge that by signing this agreement that I have been given full opportunity to ask any & all questions I might have about obtaining a tattoo, the process, healing, the business and the staff, and that all of my questions have been answered to my full satisfaction.
● If I have any condition that might affect the tattoo process or healing of the tattoo, I will advise my artist, including ALL diseases, sicknesses or illnesses I currently have.
● I am not pregnant, I do not suspect that I may be pregnant & I am not nursing.
● I am not under the influence of ANY drugs or alcohol. do not have any Medical or Health issues including but not limited to: Acne, Keloids or a tendency to scar easily, Eczema, Psoriasis, Freckles, Moles, Sunburn in the area that is getting tattooed, &/or ANY infection, ANY rash or ANY skin irritation anywhere on my body.
● I have advised my artist to any allergies to metals, latex, soaps & medications.
● I acknowledge that infection such as HIV, Hepatitis C and Hepatitis B is always possible as a result of the obtaining of a tattoo, particularly in the event that I neglect to follow proper and recommended aftercare.
● I agree to follow the instructions given to me by my tattoo artist, and I agree that any complications that arise are due to my negligence.
● I agree that any touch up work required due to my negligence, will be done at my own expense.
● My artist has advised me on location and expected results of healing the tattoo and I understand that variations in colour or design may exist.
● I understand that tattoos on hands, fingers, knees, feet or elbows are not guaranteed and touchups will be done at my own expense.
● I understand that if my skin is dark - colours in the tattoo will not be as bright as they appear in lighter skin tones.I understand that any skin treatments, laser hair removal, plastic surgery or other skin altering procedures may result in adverse changes to my tattoo.
● I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change, remove or cover my tattoo.
● I do not have any mental, physical or medical impairment or disability which may affect my well being as a direct or indirect result of my decision to get a tattoo.
● I have eaten in the past 4 hours & I will let my tattoo artist know immediately if I feel sick, light headed or need to take a break or stop.
● I am over 18 years old or I am over 16 years old and my Parent / Legal Guardian has consented for me to the tattoo.
● I have truthfully represented to my tattooer that the obtaining of a tattoo is by my choice alone.
● I consent to the application of the tattoo and to any actions or conduct of the representatives and employees of the tattoo shop that are reasonably necessary to perform
the tattoo procedure.
● I consent to ALL terms laid out in Vesna Studios' Policy and that if I have misrepresented ANY term in this agreement that I, alone accept full responsibility of any and all consequences of my actions.
● I agree to release & forever discharge & hold harmless Vesna Studio & all independent contractors or employees from any claims, damages or legal actions arising from or connected in any way to my tattoo, or the procedure and conduct used in my tattoo.
